About the Forms App

Do you need a structured, standardized way to collect information and signatures? Use Boardable's forms app.

The forms app allows account admins to collect information from users through two form types, governance forms and profile data forms.

Suggested Uses

  • Use governance forms to streamline annual compliance and board acknowledgment tasks.
  • Use profile data forms to maintain accurate and up-to-date board member information.
  • Use templates to save time and ensure uniform data collection.


Governance Forms


Governance forms allow administrators to assign documents to users for review and/or signature. These forms are suitable for collecting essential compliance and board governance documentation such as:

  • Conflict of interest disclosures
  • Board agreements
  • Policy acknowledgments

Governance forms include multiple input types such as text fields, initials fields, and checkboxes. All submitted data is saved within the user's profile for reference and recordkeeping.


Profile Data Forms

Profile data forms are designed to automatically update user profile fields in Boardable based on submitted responses. These forms help maintain up-to-date records with no manual data entry.

Administrators can deploy profile data forms to collect:

  • Full board member profile information
  • Diversity, equity, and inclusion (DEI) details
  • Basic contact information

The app includes four pre-built profile data forms:

  • Standard fields template: Collect basic user information such as name, role, and job title 
  • Full Boardable profile: Collect information for every profile field in Boardable, including non-essential details such as social media profiles
  • DEI information: Collect demographic information  
  • Basic contact information: Collect email, phone, and location information


Create a Form

  1. Select the app switcher in the upper left corner, and then select Forms
  2. Select the appropriate tab, Governance forms or Profile data forms.
    1. If you're creating a governance form, select New Governance form, and then upload the form you want to distribute. 
    2. If you're creating a profile data form, select Standard Fields Template to create a new form, or select a pre-built template.
      1. Note: Pre-built templates cannot be edited, as indicated by a 🔒 icon. 

Tip: Any form you create can be saved as a template and reused across the organization, promoting consistent data collection.
